I have been using this form for years since my MS. You should already have your SAIS account with you. Download and open the form and FILL OUT all the details.
You need to prepare this before the semester ends as they already hold a pre-registration for the next semester. You should have checked the schedule of the courses and consulted your adviser prior to submitting this. Also, check the professor of the course as he/she needs to sign. If you prepare and submit this during the pre-registration period, you should not have any issues during the actual reg period.
Funny how Sir Li would complain about how other graduate students in UPLB failed to fill out all the necessary info in the COI form (consent of instructor). This is too basic to miss out. Heads up, please.
Prepare the following info:
complete name
student number
SAIS ID
course code
units
specific section in SAIS (use search function in SAIS)
Semester and school year
Professor's signature and date
